Please complete this form if you live within the Hull City Council boundary and you want to apply for a new blue badge or renew an existing blue badge that is about to expire.

If you are terminally ill, please do not complete this form. Contact your consultant, Macmillan nurse, doctor or palliative care nurse who will liaise with the Blue Badge Coordinator to fastrack your application.
If the badge is for a child aged under three or for a child who is not in receipt of HRM (Higher Rate Mobility component) or PIP (Personal Independence Payment) scoring eight points on 'moving around' contact the CFDT (Children and Families Disability team) on 01482 448 879. Do not complete this online form.

You will need to answer a series of questions to establish whether you are eligible for a blue badge. If you are completing the form on behalf of someone else, please enter their details, not your own.
You will need to make a £10 payment online in this form if your application is successful. Please make sure you have a credit or debit card to hand.
